Elevate/FDA requirements:

Your donor will complete a full suite of screenings to meet FDA regulations and Elevate’s elevated standards. This includes a comprehensive physical exam, psychological evaluation, semen analysis, genetic counseling, and an advanced genetic panel (via Invitae, Sema4, or Counsyl). He’ll also be tested for all major STDs through a full infectious disease panel.

For non-directed donations, the sperm is quarantined and the donor is retested six months later, per FDA guidelines. In directed donations, that additional testing period is not required.

In accordance with guidelines from the American Society for Reproductive Medicine (ASRM), sperm donors must be between 18 and 39 years old. However, to ensure the highest standards of sperm quality and long-term health outcomes, Elevate sets a more conservative age limit, typically capping eligibility at 34 or 35.

The medical screening is a comprehensive evaluation of your donor’s overall health. It includes measurements such as height, weight, and blood pressure, along with a full physical exam. Blood and urine samples are collected to run the FDA-required infectious disease panel, which screens for a wide range of conditions—including sexually transmitted infections—to ensure the highest standards of safety and compliance.

Donors provide a sample in a private, comfortable setting at a certified clinic. The sample is then evaluated in a laboratory to assess sperm count, motility (movement), morphology (shape), and overall quality. This ensures that all potential donors meet the highest medical standards before moving forward in the process.

Genetic carrier screening is performed using a blood or saliva sample to determine whether a donor carries any inheritable genetic conditions. The screening looks for hundreds of potential conditions—many of which may not present symptoms in the donor—ensuring the highest level of safety and transparency for your future family.

As part of the screening process, donors are required to provide detailed medical history for at least two previous generations of their family. This helps identify any patterns that may suggest a hereditary condition. While not all medical history concerns are disqualifying, a significant family history of genetic disease may make someone ineligible to donate. Our goal is to prioritize the long-term health and well-being of any future children.

Psychological screening is a vital part of the donor evaluation process. Donors meet with a licensed mental health professional who assesses emotional well-being, mental health history, and overall readiness to participate. The evaluation also explores the donor’s motivations and ensures a clear understanding of the legal, ethical, and emotional aspects of donation. All assessments are conducted in accordance with guidelines set by the American Society for Reproductive Medicine (ASRM) and the Society for Ethics in Egg Donation and Surrogacy (SEEDS), ensuring every donor meets the highest standards of care and responsibility.
